Application $50.00 (Non Refundable)
(Upon acceptance of your application a separate invoice will be sent for the certification fees)
Level I Certification $450.00
Level II Certification $350.00
Level III Certification $275.00
Note: All certification levels expire after five years of issuance. Re-certification requires evidence of continuing education.
Information for re-certification will be sent 90 days prior to the expiration date.
